Adult Day Health Care Services
Adult Day Health Care of Mad River is an enrollment program offering a wide variety of professional services to meet the varied needs of our participants. Participants are generally enrolled for at least 6 months of regular attendance, but many continue to attend our program for 5 years or even more!
Respite Services
For those who may not be ready to commit to our standard enrollment program, or who only need respite care on a more occasional basis, this service may be for you. The sign-up and approval process is shorter, and once service has been approved, clients may attend for anywhere from 1 hour to 5 hours in a day with as little as 24 hour advance notice.
Participants in our Respite Program have access to all of the services in our enrollment program, but with greater flexibility to meet your variable needs.
This is a free benefit for qualified Medi-Cal patients and only $33 per hour for those who wish to pay privately.
Enhanced Care Management
This benefit is for Medi-Cal eligible clients who need some extra support to avoid frequent visits to the Emergency Room for non-emergency services and those who are at risk for placement in a Skilled Nursing Facility due to un-met healthcare needs.
Care Management is provided by either a Social Worker or Registered Nurse, and services may take place in our center or in the client’s home.
