Getting Started

- Sign a Release of Information Form so we can obtain records from your doctor
- We will review your records and confirm your eligibility for enrollment
- Our Social Worker will visit your home and explain more about our program
- You will be scheduled for two free visit days to fully determine if our program is right for you
- After your visit days, we will schedule assessment days and complete the enrollment process
